Translation to Practice® (t2p) - 2023 Family Medicine POP!
Translation to Practice® or t2p® is an AAFP-approved activity supplement available for selected 2023 Family Medicine POP! activities. It allows you the opportunity to translate learned concepts from the activity into your practice and report back on the results. Learners who choose to participate will be eligible to earn two additional Prescribed credits, per CME activity, upon full completion of the t2p process.

Participation and Credit:
There are three steps associated with participation in Translation to Practice®.
- Commitment to Change: Complete a commitment to change statement indicating what change you would like to implement.
- Implement Change: Strive to implement the learned change into practice.
- Post-Activity Evaluation: Evaluate whether the implemented change resulted in improved patient care and assesses any barriers or challenges that were encountered. *This evaluation opens 15 days after the Commitment to Change statement is first accessed.
Learners are eligible to earn two (2) AAFP Prescribed credits per CME activity.

The AAFP has reviewed 2023 Family Medicine POP! and deemed it acceptable for AAFP credit. Term of approval is from 8/25/2023 to 11/25/2023. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
AAFP Prescribed credit is accepted by the American Medical Association as equivalent to AMA PRA Category 1 credit(s)™ toward the AMA Physician's Recognition Award. When applying for the AMA PRA, Prescribed credit earned must be reported as Prescribed, not as Category 1.
Learners can earn up to 2 AAFP Prescribed credits for participation in EACH Translation to Practice® (t2p) survey from 2023 Family Medicine POP.
