Translation to Practice® (T2P) - 2022 Family Medicine Clinical Forum
Thank you for participating in Translation to Practice®. In this follow-up survey, you will be evaluating if you were able to achieve the practice change indicated in the initial survey and report on the results of your implementation.
Participation and Credit:
There are three steps associated with participation in Translation to Practice®.
- Commitment to Change: The learner completes a commitment to change statement indicating what change they would like to implement.
- Implement Change: The learner implements the learned change into practice.
- Post-Activity Evaluation: The learner evaluates whether the implemented change resulted in improved patient care and assesses any barriers or challenges that were encountered.
Learners are eligible to earn two (2) AAFP Prescribed credits per CME activity.
Translation to Practice® is a product and registered trademark of the AAFP.
The AAFP has reviewed 2022 Family Medicine Clinical Forum, and deemed it acceptable for AAFP credit. Term of approval is from 4/22/22 to 10/22/22. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
AAFP Prescribed credit is accepted by the American Medical Association as equivalent to AMA PRA Category 1 credit(s)™ toward the AMA Physician's Recognition Award. When applying for the AMA PRA, Prescribed credit earned must be reported as Prescribed, not as Category 1.
Learners can earn up to 2 AAFP Prescribed credits for participation in EACH Translation to Practice® (T2P) survey from the 2022 Family Medicine Clinical Forum.
